Yet another “tales from healthcare” story (I’m so tired):
Met with a physician who advertises himself as different. Welcomes discussion and questions and shared decision making. Looks great on paper.
Reality is he’s a paternalistic asshat who reacts to questions with dismissal, and then if you push, scare tactics to shut you down. Just be the asshole we expect surgeons to be. This is exponentially worse.
He ignored my attempts to explain my background in research and academic medicine as he gave blanket advice on information sources. Rather, relying a binder of printed materials that also exist on his website. He asked me no questions.
In his binder, which I threw away, he put his CV. So I decided to take him up on his open offer to email him because he “encourages” this, per his website-binder.
I was cordial and thanked him for his time, acknowledged his expertise, and then sent him my CV so he can understand my preferred communication methods going forward. My CV blows his out of the water on all fronts, and he’s probably got 20 years on me in academia. His reply: thank you for sending, very impressive. No acknowledgment of my requests or concerns.
I want to reply “I know” so badly, but I am relying on him to fix a problem. And he is my 2nd opinion, and considered “an elite surgeon” by people I know and respect. So he’s basically an auto mechanic at this point. Fix it and fuck off into the sun.
So this is where US healthcare is. Sending my fucking CV to try to get doctors to listen and have real conversations not mired in bullshit percentages and dismissal.
Because the way his tone shifted when I mentioned a particular clinical trial was stark. I assume he thought oh yeah another one who reads research she doesn’t understand. Motherfucker, I understand. :elmo_fire:

I had to show up to the medical conference yesterday to say hi to a few folks and do some recon for booth planning for next year for a non-profit I work for part time. I was there maybe 2 hours and then proceeded to walk the earth to try to cleanse my soul, apparently. I had no idea how far I’d walked until I finally found a small place to grab some dinner around 9pm. I really need to find a way to stop attending these things.

After pushing back on the interventional radiologist's take on the importance of biopsies, I got her to acknowledge that "it's not just come in for a biopsy" and that there definitely can be "fatigue" from the process. To which I shot back "Fatigue? Is that what we're calling it?"
In her defense she was somewhat open to what I was saying about my particular shituation and acknowledged mistakes were made and stated she would incorporate my entire clinical history into her report after it was ignored by multiple people at 2 separate medical centers.
Fuck this whole process.

Attended an "AI training" today where someone rapidly fed prompts into Claude to show how it can help with policy work like reviewing and summarizing lengthy documents and shit. At the end, during Q&A, I asked what the presenters thought about the fact that the people on the other side are also going to be using AI to streamline their processes so now we just have AI talking to itself.
One acknowledged that's probably happening and it's an issue. The other went on some tangential explanation about how AI is here and we have to learn to use it properly and how it helps her college student kid study and blah blah blah. Seems I hit a nerve. My work here is done :elmo_fire:

I’ve only seen a couple of people in my career with true narcissistic personality disorder. And I think it’s only because they came to me for a chronic medical problem, not to gain insight into other issues. People with NPD don’t go to therapy because there’s nothing to fix, in their minds. Therapy is generally contraindicated for NPD because they use it to air grievances about how others have wronged them. It becomes apparent very quickly when you have someone with NPD sitting across from you. And they will quickly fire you if you don’t go along with their greatness.
After declaring he was done with treatment after maybe 4 visits, one guy I saw gave me a jar of mustard. I clearly still think about that from time to time 15 years later.

One of the tidal pools on the trail to Queens Bath. In the summer the north shore is relatively calm, so you can make your way along to the enclosed pool to swim. The water is such an amazing color and you can often spot sea turtles riding the waves nearby.

After discovering an ex boyfriend was arrested in Florida on 3 separate fugitive from justice charges, I have intermittently dug around more into his legal history since he ghosted me in 2001 and oh my god it's fantastic.
Breach of contract. Fraud. Evictions. And that's just in Cook County (Illinois). Plaintiffs include that ultra high end boutique he was consulting for in Chicago (Ikram) and OpenText.
More recently, for about 4 years he owned a resale shop in a town outside of Tampa, Florida in the same strip mall where he kept his PO Box. I can only assume this was a front.

I spent several hours yesterday trying to trace my great grandparents' lineage to see if I'm eligible for Polish citizenship. Because, you know, USA is gonna USA. Unfortunately I hit a dead end. They left when Poland wasn't actually a country yet (1906-ish) and were naturalized on the 1930 census form that I found. In theory they could have naturalized after 1920, which is one of the rules to use ancestry for Polish citizenship, but I'd have to get naturalization records. Even then, I need a birth certificate from Poland for at least one of them.
But how cool is it that I found that document? I did end up on Poland's records site and birth records from 1888 are just photographs of long paragraphs of who knows what language in these big old books. I'm kinda surprised they survived WWII.
Also, why so much goddamn cursive in these times? Can't decipher half the stuff written down. I think I found my great grandfather in the Ellis Island records, but no luck with great grandmother.
Oh well, it was a fun exercise.

Yesterday our keynote speaker was a “strategic futurist” so yes of course they talked about AI for 45 minutes. During a conversation at my table, people shared how they use AI and I asked one person, who was using it to summarize social chatter on the internet about certain topics, how you can vet the information given. At first I think she felt defensive, but as she talked through it you could see a realization like oh shit I really can’t.
Then toward the end of the day we had to fill out a form for compliance reporting (pharma has to report if they gave me food) and since I’m a licensed healthcare provider I have to report my NPI number (national provider identifier). I don’t have that memorized so I googled it, something I’ve done for years to quickly find it, and the AI summary said there was no information linking my full name and degree to an NPI. But if I scrolled past the summary the first website preview listed my NPI.
So I showed the woman next to me like hey here’s an example of how AI gets it completely wrong. She was floored.
It’s bad out there, folks.

A shoutout to the Chicago TSA workers who fuckin showed up to OHare today. First day of spring break for most schools and it’s the busiest I’ve seen it. Long lines but moving pretty fast. Because TSA was fully staffed (at least in terminal 1).
Best part, the United employees were using the wands they use to guide planes to direct people to the end of the lines 😂

Unnerving encounters PSA:
Yesterday while walking my dog I had some dude walking toward me who honestly made me think I was going to have to throw down. We were walking down a street without sidewalks, against traffic as one should (there were no cars it’s a quiet neighborhood road) and this guy is approaching on the same side. I start moving over because I’m on the phone and just want to keep it moving.
This guy starts walking toward me, staring kinda like Charles Manson. Wide eyes, not blinking, no emotional affect whatsoever. I stare back and he shifts his gaze to my dog. Axel is about 70 lbs, half boxer half mountain curr, mostly friendly but has barked at a few men while on walks in the past. This guy keeps slowly walking, staring and when he got to about 8 feet away I said “he’s not friendly” pretty sternly and never breaking my gaze at him. He pivoted and kept walking past us. Never said a word, never changed his face. If he would’ve tried something Axel likely would’ve jumped at him, I don’t think he’d bite. He’s never bit anyone. But I’m not sure I’d have cared if he did. If I had to guess he was high on something.
So yeah, don’t fuckin do that to women.

So, when I got married I changed my last name mostly because Mr. Nay's surname is super simple and my maiden name is a pain in the ass I had to always spell and nobody could pronounce, etc. At first I moved my maiden name to be my middle name, mostly because I was graduating from grad school, working on getting licensed, etc. and was advised this would make things smoother. I eventually dropped my maiden name and went back to my original middle name. This is what's on my US Passport.
When I voted in the primaries earlier this month, I noticed my registered name was the First Maiden Married, which does not match my passport. You know, the ID I'll need to present if the SAVE act is passed. So I went online and requested a change to my name on the voter rolls. This is the subtle shit a lot of people may miss should that stupid legislation be passed. If I didn't catch it, I would have rolled up in November and been turned away.

The other day, 14’s school hosted a “Marines challenge” during gym classes and she clocked that bullshit immediately. I’m guessing it was local recruiters talking up the few, the proud, the marines. They had a form you had to fill out with your contact information to participate. It was voluntary and 14 declined, calling it recruitment propaganda. Her teacher tried to explain it and 14 was like nah, bruh. Pass.
