For real failed-htlc patterns on lnd, the GetForwardingEvent stream gets you partway, but you end up parsing debug logs, since not every failure surfaces in the API. One caveat on all of this: your failure rate is a view through your own channel set, not the network as a whole. Compare it against its own baseline.
