I think the main answer is because they tend to be poor dirtbags with disabilities and lead otherwise very stressful lives.
For the better off (which I think are rare), I think it's just a continuation of the selfish culture of taking care of yourself first. They rationalize purchasing their video games, concerts and whatever as "self-care".
