One time I planned a loop hike in Oregon in June. Late snow year. Day two, it was sleeting. The forest was waist-deep snow. I gave up, pitched a tent, and spent the night eating about 3 days worth of food to stay warm despite a zero degree bag, layers, and parka. It was just plain frigid and wet. So please my EM friends, stop seeking for the easy out. SIT down. Take a good history. Do a decent exam. Obtain appropriate testing. Finally, if you have a picture of anxiety, having ruled out lethal processes, present your decision making to the patient in a way that makes it crystal clear that you don’t know for sure what’s happening. Leave space for error, change, and re-evaluation. Validate the visit. Damn near all ER visits are appropriate — it is the system that is not.

My boss saw a guy propping himself up rigidly on his elbows, eyes wide as dinner plates. She ordered testing. He had an aortic dissection and was trying to die.

She’s also a PhD Psychologist. She would know. What she did have, is over-replacement of her thyroid hormone, which is common. Her dose just needed an adjustment. The PA that saw her perpetuated a (literally) textbook case of systemic bias in medicine that kills women.

I will always tell my patients who clearly have anxiety “your tests today look great, and so I have ruled out the obvious life threatening causes of your symptoms. Medicine’s a big puzzle. Sometimes I don’t have all of the pieces. If you are feeling worse, or get a new symptom, please come back immediately. You know your body better than I do.”

I always do labs, and get an ECG.

I never, ever, diagnose anxiety in the ER. I rarely state it in my chart in a way that might get snatched up by crawlers or by other providers doing a quick review. It creates bias in the chart forever.

For example, when a person with severe dementia tells you a very plausible story about how they arrived at the ER today, but five minutes later they tell a completely different tale, they are confabulating. Their brain knows they should have a story, so they just start filling gaps. LLMs do this. It is unpredictable, and extremely hard to detect in a written result.
