Soviets were doing research into chimera pathogens in the Biopriparat weapons program that used plague. After the Soviet Union fell Dr. Kanatjan Alibekov (Ken Alibek), the former deputy director of Biopreparat—revealed to Western intelligence that Soviet biochemists had actively mastered the creation of genetic "chimeras".

So encephalitis is spliced into the plague DNA. You catch plague and either die from it without treatment or are successfully treated for it with antibiotics. But when the plague bacteria are broken down this releases the encephalitis viral payload. And because it was delivered by the plague pathogen that had already spread deep through the host's systems, it is already behind the blood brain barrier and there is no defence.

So almost 100% fatality whether you are treated or not in the end. Nothing like it exists in nature.
