I had long suspected my synpase database was missing an index. I had previously checked to confirm that the indexes that synapse creates for some tables I knew are problematic existed. However, figuring out the expected synapse DB schema is quite hard.
Anyway, I've finally used the strong tools and found the missing index and now synapse is suddenly much faster and responsive. Weird how that works.
I think I probably lost that index a couple of database upgrades/migrations ago^^
