There are multiple ways to do it, but -- assuming you're looking for something you can automate as a rule -- I'd recommend small quote mark/big letter, extra credit if the big letter is at the left margin and the small quote is floating a little further out. Extra, extra credit: use a different typeface for the quote than you do for the text to avoid the alignment looking subtly wrong.
