This is a very simple cost/benefit concept that most people don't understand because they don't know the facts. It's cheaper to just give away care to people who might be gaming the system, because it would be more expensive to try to prevent that, and every time it's investigated it's found that actual incidence is much lower than feared. It's just not WORTH addressing.
