Why don't you always see every reply to a note on Nostr?
Everyone publishes to the relays they chose for writing. They also announce other relays where they read: that's where others should send them replies and mentions. It's their "inbox" (NIP-65).
The problem is that your client usually only asks your own relays. If someone replied on a relay you don't use, that reply doesn't exist for you, even though it's out there.
Quill now also asks up to 3 of the note author's read relays when you open a thread. If it finds replies your relays didn't have, you'll see a note about it under the thread.
Tip for everyone: check your relay list (kind 10002), and keep your read relays few, stable and open to others writing to you. That way replies actually reach you.
