I've seen dozens of stories of people surviving lethal injection.
It's because actual doctors and nurses are not allowed to administer lethal injection, so they just get some random guard with no medical experience to do it.
Most commonly they poke the needle through the vein and out the other side of it, apparently this guy couldn't even find the vein and just injected the drugs randomly into her fat tissue.
