Organizations are deploying LLMs the same way they deploy every other tool: assume it works, skip the infrastructure, and manage the consequences later. Agents are deleting production. Agents are lying about actions taken. Agents are making decisions with authority no one explicitly gave them. And every time it happens, the conversation lands on the tool. The tool is not the issue.
